Health Foundation of La Porte Awards Grants to Northwest Health – La Porte

Northwest Health – La Porte was awarded two grants from the Health Foundation of La Porte during the last grant cycle of 2025. The grants were announced on September 25.
The Direct Patient Access program provides $30,000 to serve community members who do not have adequate funding for prescribed medications, transportation to and from medical care, nutritional supplements, expenses related to supplies and durable medical equipment, and other needs identified on a case by case basis. The hospital expects to support 60 La Porte County residents with these funds.
The Foundation also awarded the hospital $4,700 to purchase supplies and provide staff education for a Perinatal Bereavement Program supporting families who experience the loss of a baby during pregnancy, at birth or shortly after birth. The program provides emotional support and resources to bereaved families during this especially trying time.
The hospital is also applying grant funding from The Health Foundation of La Porte to fund a portion of its Community Caring Connections service, a heat illness response program, perinatal case management and the Lil’ FISH Club.
Health Foundation of La Porte (HFL) is a private, independent foundation located in La Porte, dedicated to empowering La Porte County residents to live healthy and well. HFL has invested more than $49 million in the La Porte County community since November 2017.
